My patients in New York are suddenly afraid to eat their salads. But that reaction pales in comparison to what's happening in Europe, where consumers are coping first-hand with the sudden outbreak of a highly toxic strain of a known bacteria, Escherichia coli, more commonly known as E. coli. So far at least 24 people have been killed and thousands, sickened. People are panicked and worried every time they lift a salad fork. This is a common reaction. Whenever there is a sudden, deadly infectious outbreak, policy makers and consumers tend to personalize the risk. E. coli is an invisible, microscopic killer, which ramps up the fear quotient. It is difficult to wash the bacteria off of vegetables or your hands, compounding the sense of vulnerability. Add newspaper headlines trumpeting the "most deadly outbreak on record," and the juices really start to flow. Little wonder Russia banned imports of all produce from the European Union, or that Germany is pointing the finger at Spanish food producers, and vice versa. What are the facts? Though there is a hunt on to determine which vegetable is the source of the disease, most of these outbreaks originate in cows. Most farmers around the world feed cows grain instead of grass, which creates a more acidic environment conducive to bacteria in the animal's intestines. Before the cows are slaughtered, they are routinely treated with antibiotics to ward off infections. This is a normal and desirable process. The problem is that in making food safe, we can also create resistant bacteria with a Darwinian survival advantage. This mutant bacteria passes into the cow's manure, contaminating crops either directly or indirectly via rainwater or faulty irrigation. It really makes no difference whether the offending crop is cucumbers or bean sprouts or spinach; what matters is the process that put the bug there. The current outbreak of E. coli is distinct because of the power of its toxin. Some variants can cause bloody diarrhea and fever, but this particular toxin is so powerful it has caused kidney failure in more than a third of cases, a very unfortunate development because that often leads to dialysis and is irreversible. This disturbing trend toward a more potent E. coli toxin was noted in the European Center for Disease Prevention and Control's Annual Epidemiological Report on Communicable Diseases in 2010. So though the overall fatality numbers are still small, this strain of E.
